Here is an article with a guy that has no debt and 1.6 million saved and a budget of 60k asking if he can retire at 52.

They talk about healthcare costs and not a single mention of healthcare.gov

The article does mention to "[l]ook at the health insurance marketplace."

It does spread FUD re: insurance rates. At $60k/year, there's a good chance that the email writer would qualify for subsidies, even as a single person. With $800k in a brokerage account, there's probably room for some Roth conversions too.

Geographic arbitrage for medical care is a great option.

Also, and this may be a bit unseemly but, go get the bronze/silver ACA plan and if you do have some sort of catastrophic medical bill… stiff/slow-roll the hospital.  The ER has to take you, they frequently bill you after the fact with insurance for many in-patient items.   Most hospitals will accept a marginal minimal monthly payment you can really stretch medical debt out for a LONG time.  Plus it doesn't accrue interest.

Worst they can do is ruin your credit score (who cares when you are a debt free millionaire) and I guess sue you (if all your assets are in retirement accounts and/or your primary residence, and you have no W-2 income... they'll be suing a rock).
