What kind of tax form for a nanny

First off, I don't know if this should be in the "Taxes" sub-forum or the "Ask a Mustachian", feel free to move if I misplaced it.

My wife was a nanny for a friends child for 9 months of 2016.  The parents of said child are using/used Dependent Care FSA funds to pay her.  We are reporting the money as income, but want a "form" as proof to put in our tax folder.  Does she need a 1099? What kind of 1099?  I have a spreadsheet of her earnings which ties to the parent's spreadsheet, but I want something more formal.  Any thoughts?  Googling started talking about W-2's, but I feel that was overkill.  Gross earnings was roughly 6k.

Any help greatly appreciated.

The point of view is from the household employer, but it gives you the answer. (The parents need to send you a W-2.)

You should be able to point the parents/employer to the post if they question need for W-2.
