Documents to file with 1040x?

Doing my taxes for the third time ever, second time this year.  I decided to re-characterize Roth contributions to traditional IRA since tIRA would be deductible.  I've never filed a 1040x before and I have a simple question.  Do I need to file a revised form 1040 showing the change on line 32 (IRA Deductions)?  Do I need to include the prior 1040, the new 1040, and the 1040x? 

The instructions are really unclear, stating "Assemble any schedules and forms behind Form 1040X in order of the 'Attachment Sequence No.' shown in the upper right corner of the schedule or form", but of course the Form 1040 upper right corner has no attachment sequence No.  Also, "Do not attach correspondence or other items unless required to do so."  But how do I know?

1040X and any supporting schedules (like 1099s).

Because the 1040x requires you to report what you originally included on your 1040, there is no need to include the original (they would have the information in their system).  A new 1040 is not required -- just a 1040X which reconciles the specific changes back to your original return.

Thanks for the reply, Trudie.  I was thinking the old 1040 is not necessary, but was unsure about a revised one in addition to 1040X.

This change nets me over $1k in additional refund, so worth the confusion and extra work.  Plus I learned how to do something I hadn't before, though I hope I never need to again.  I will definitely be doing any re-characterization prior to filing in future years, but this year I only learned it was possible on this forum AFTER I had filed.
