Insured by Anthem- Get $300-$700 in gift card incentives

If you have Anthem insurance, register your account and see if you have a Wellness Toolkit.  You can earn $300 for completing health modules, a questionnaire, setting and achieving health goals, certifying as a non smoker, going in for an annual exam etc.  You can also get $200 in gym membership reimbursement by going to a gym 36 days per 6 months, up to $400 for the year.  Has to be a paid membership type of gym (fitness facility, rec center, cross fit, climbing gym, yoga studio etc).   I signed up a couple days ago and have $200 in gift card vouchers en route already; pretty easy stuff.

This must be something your employer adds to the plan.  I have wellness toolkit but no offers of anything.

Hmm, looks like you're right.  Looks like employer sponsored plans have 4 options for wellness benefits and this is 1 of them.
