How long have you been FIRE'd?  Less than 5 years.

How old are you?  51, 48, 14, 12, 10
How long have you been FIRE'd? Depends how you count. Let's say 2 years. Still do some part time work. Been self employed for 19 years. Buying our own insurance for 11 years.
Have you had what you characterize as a major (read: expensive) medical issue in retirement? Yes. We hit max OOP for Mrs. C last year.
How do you handle healthcare?  (e.g., What care do you have and how do you purchase?) ACA
If you purchase, what do you spend? $1300/mo w/o subsidy, Bronze, family of 5. We may get a subsidy this year, not counting on it. Last year we spent $14K on insurance, $16.6K on medical/dental out of pocket.

Using COBRA for the 18 month duration I am legally entitled too, costs approx. $815 / mo for spouse and myself.

If you purchase, what do you spend?
Will be purchasing private health insurance plan thru ACA when COBRA runs out, looking like approx. $1100/mo for spouse and myself.

No

How do you handle healthcare?  (e.g., What care do you have and how do you purchase?)
ACA bronze

If you purchase, what do you spend?
Insurance: ~$1400/ month before subsidy (Washington Post called out our county as having most expensive ACA plans in US) and ~$200/ month after subsidy
Out-of-pocket: $500 to $1000 most years

Other notes:
I work very hard and proactively at being super healthy including: good nutrition, heavy and varied level of exercise, low stress. I aim for physical therapy for sports injuries as my only significant medical cost!
