Dental Insurance Claim for Crown EOB question

My previous post didn't show up right, so here's another one.

I got an EOB for a new crown on a cracked tooth. The EOB is requesting 'please resubmit with a
narrative of medical necessity'. At this point, the EOB says they are not approving anything.
I called the dental office clerk and she said not to worry about it,  and she would write a letter to
 the insurance co and provide the narrative. 

To complicate things, I had a filling put in the same tooth a month before the crown, but the filling
 came out.  Could that be part of the problem? They ins co paid the claim for the filling, by the way.
