When are 401k fees applied?

I finally found where the fees are listed for the former employer 401k plans that are eligible for rollover.  There are expense ratios (anywhere from .07 to 1.06%) and management fees (from .06 to 1%). 

When are these fees removed?  i.e. should I rush to get these rolled over before the end of the year? 

Do IRAs charge both expense ratios and management fees? or is that management fee a "feature" of the 401k plan?

The IRA doesn't charge anything, it's the fund which you invest in which charges fees. Your 401k plan's documentation will outline all the plan fees and define all the terms for you.

If I were in your shoes I would rollover all assets which are eligible for rollover from the 401k into an IRA ASAP. You'll be able to control costs and fees much easier that way.

They will charge fees as much as possible and then try to keep the hidden.

I have to pay $50 to rollover a 401K from a former employer. The fund has been charging me several hundred per year.

In one of the 401k accounts, we could switch it all to FXSIX, which has combined expenses of .065%.

In a Roth, also at Fidelity, we have money in FUSVX, which has combined expenses of .12%.

As far as I can tell, they're the same fund, but lower cost inside the 401k.

The fees are removed continuously from the fund NAV. You'll never see an ER fee on your statement.
