Name/Inception Date Asset Class Category 1 Year 3 Year 5 Year 10 Year/LOF* Returns As Of Bench- mark Gross Expense Ratio**

I would do some combination of those two (maybe 85-15 large to small cap), or 100% to the large cap one. Those expense ratios are pretty crazy though.

Thanks and yeah I thought these were pretty crap options. My last employer had a decent Fidelity 500 Index Fund and I went with that. At least my thinking is on track.

That's where my eye goes too. An 80/20 mix of those would probably be pretty similar to a total US market fund.
The target date funds, like BLKRK LP IDX 2055 IS (LIVIX), would also be a fine option. All the funds are a little higher expense than the ideal versions of other funds, but these target date funds are no more expensive compared to your other options than other target date funds are compared to other options one might find as alternatives to them. For example, in an IRA you might be able to get VTSAX for 0.04% expense ratio (or whatever it's at now) and the vanguard target date fund for 0.15%, and get some extra bond and international exposure. For your options you're looking at an even smaller difference and would probably also get some extra diversification without having to rebalance.

Thanks all. This is helping me make a decision for sure.

I would actually opt for the target date fund... are you retiring 30 years from now? If so you'd pick this one:
BLKRK LP IDX 2050 IS (LIPIX) 05/31/2011 Blended Investments N/A -7.87% 6.61% 5.23% 6.70% 12/31/2018 Show 0.24%
Most of your choices have high expense ratios, which narrows your choice to the two U.S. funds with 0.20% expense ratios and the target date funds ranging from 0.21% to 0.24% expense ratios. Paying 0.04% more gets you international, which is a good trade off. And while this target date fund has almost no bonds, you do get a tiny percentage of bonds as well. Plus the fund changes allocation over time, so you don't need to deal with rebalancing.

assuming you won't wash sale. Large Cap, Mid Cap, Small Cap, and international. With a value tilt.
I'd go 30 20 20 30 in the same order. Or even 25, 20, 15, 40. But that is me.

OP isn't going to have a wash sale on mutual funds only owned inside a 401(k) and nowhere else.
Also, everyone else is mentioning specific funds and expense ratios, which you don't mention.
