Have you tried a request, in writing, for their contribution (with appropriate account details)?

Harder to get "brushed off," and with the year over, no excuse for "we'll have to see how the company does this year" (and then hoping you forget).

It is in no way optional. There are a few different options but its is mandatory they follow which ever option they choose. Who is the plan provider? I have no idea who you report issues too. I think I would start by asking the plan provider.
