Couple vinyl flooring questions....

1. What do I do about the transition on this subfloor?
2. I used regular thinset to try and level the floor in my basement: retrospectively I would have used self-leveller all over the thing. I have several boards lifting a bit, my plan was to glue these down and put a weight on it to adhere better. Is there a better option?

For the transition you can cut a piece of flooring and attach it to the vertical using Liquid Nails, then put a cap over the outside corner and a round over on the inside corner. Most brands sell matching trim pieces, though its typically a special order from HD or Lowes and can take 4 weeks in a non-lockdown time period (as me how I know...)

As for the lifting.... yeah, you can use liquid-nails to help hold those down.  I had something similar (live and learn)... 
In retrospect the most important thing is to have dead-level floors.

I'd custom rip a tapered oak threshold.

I just got done making four different threshholds for my flooring project. Since I was installing hardwood, I just made them from leftover pieces of the flooring. But since you have really thin stuff, I would just cut down a piece of oak like mentioned above and stain it to match. If you have a router, you can round over the edges a bit. If you don't have any equipment to do that sort of thing, go visit the wood trim section of your closest big box store. I have made threshhold out of door jams and other pieces of trim that I simply had to cut to length and stain.

DH just did our stairs (and kitchen). Same flooring and adhesive for the vertical pieces.  Transitions were purchased from a store that specializes in flooring transitions.

The instructions for installation probably recommended that subflooring be level to within a eighth of a inch (or so).

This.  Cheaper than buying one from the flooring company, will hold up better, and you get exactly what you want.
