Did you check out plansponsor.com? They rank 401k providers by size, fees, etc.  Start up plans can be expensive, but as your plan grows, the fee should get smaller.

Some providers charge one fee for everything (annual testing, 5500 filings, document prep, payroll processing...) and some charge piecemeal, so make sure you are comparing apples to apples. 

Many companies also hire a 3rd party/consultant to help with all of this.  The name of the game is CYA.
