So I got a refurbished HTC one off amazon and I received my airvoice simcard in the mail.  I would like to keep my old number so it seems like before I do anything I need to put in a request to port my old number from verizon.  The form requires my Verizon account number and password/pin number; which I assume refers to the billing password as it is in a four digit form as oppposed to the account password which is ten character alphanumeric.

  I am hesitant to give AV my Verizon password as my family will continue to use this account.  Could I contact Verizon directly with the request to port my number?

It's the company you're switching to (not from) that you contact, so to answer the ending question, no.

Call and find out why they need it, or submit it without it and see what happens.

At least with AT&T you can change this password at any time. So you could just give Airvoice the current password, then change it after the port is complete. If the rest of the family doesn't want a new password, reverse the order: change the password, give Airvoice the new one, then change it back after the port is done.

You must give the Verizon account number and PIN info to Airvoice if you want to port your number, full stop. This is the safeguard that helps prevent people willy-nilly stealing phone numbers from other people under the LNP system. If you're concerned about handing out the PIN to Airvoice, you are going to have to change it on the Verizon account either before or after the porting process. Contacting Verizon directly about a number port to another carrier could result in you losing your phone number from a misconstrued account cancellation.
