I searched and didn't see anything on it and I even googled it. So let's say at some point I decide to work only part time, and if the place doesn't offer a 401k then what I can put my money into besides after tax investments and possible IRA?
I should mention that the work likely will be a W2 employment.

I think the reason you're not getting responses is that there isn't anything else. Whether you or full-time or part-time doesn't change that. :-(
As a physician I would think that most places you would work would have a 401k (or 457/403b) even if they didn't contribute a match. If you were self employed (not W2) you could do a solo 401k. Try googling that. Again PT vs. FT shouldn't specifically be an issue.
