Health Insurance options for US expat

I am a US citizen living and working outside the US.  Presently have health insurance through my employer.  Employment situation most likely is going to change 4Q 2017.  Preference is to continue living and working outside the US.  Trying to prepare for possible period of no employment between jobs.  Want to be sure my wife and I are covered with health insurance that covers treatment both internationally and at US care providers.  Wondering if anyone else out there reading who lives outside the US is self-insured with insurance that covers treatment both in and outside the US.   If so I have a few questions:

     --  How much are the premiums?
     --  How much is the deductible?
     --  Are pre-existing conditions covered?

It probably depends a lot on what country you're in, which you don't specify.  A google search turned this up, which might be suitable even though it's marketed as travel insurance.
