Photo program for cataloging with custom fields?

Help!
I’m trying to find a program  which works with an iPad that will allow me to capture images with specific data tags at the time of capture (see below)
. We’ve tried Adobe Lightroom, which allows us to set custom tags but it all has to be done afterwards.

Conceptually, we are tracking the growth of organisms over time. Each organism has a specific code, growth media, and “birth date”. Each time we take a photo we want that info tagged into the image file (along with date & time) so we can sort through them weeks later. Challenge is we may take 100 photos a day, and it’s unmanageable to try to add that info even a couple hours later.

I’m envisioning a photo program which works like other lot-tracing (non-image) software where for each “click” of the camera  a drop down menu pops up with pre-populated date/time fields and the user selects the other inputs right on the iPad

Anyone know of a program which might do this?

Not sure if this would do what you want but I've dabbled in the free version for awhile and it seems pretty powerful once you learn it.
