I placed my order using the referral you sent, they sent me a link in my email so I didn't need a form :-)   It auto-filled in your info as my referral so hopefully you got your credit per your "that worked out well" comment. But if not, I'm sure you'll get it soon maybe when I activate the SIM, should arrive within 7 days I think it said.
