The limit on Freedom of Speech in the USA is that you aren't allowed to instigate violence against someone with your speech, so people aren't allowed to threaten someone's physical well-being or property with their speech. However, if a person wants to say insulting things about Group XYZ, they can do that all they want. It's legal for citizens to respond to that kind of behavior with economic boycotts, calling employers, going to the media, etc., because that is the exercise of their own Freedom of Speech, so people usually police themselves about the really bad stuff. This is why we're seeing Alt-Right groups being systematically deplatformed by various internet services.

We tend to feel that we are required to tolerate some pretty hateful speech, because without the law it would be possible for someone like, say, Donald Trump and his Tea Party to come into power and then decide that everyone using non-gendered pronouns is participating in unlawful hate speech against cisgendered people. Then a bunch of folks would get locked up for their speech. As a nation, we decided to simply allow pretty much everything to avoid anybody having that kind of power.

This doesn't stop people from saying really horrible things, it stops them from saying really horrible things that would lead to violence (the breach of the peace phrase).  There have been court cases about this that have clarified it (I think MacLeans was involved in one?).
