Bad 401k's in the news

There's a class action suit going on against Lockheed, today regarding the poor choices in their 401k.

One thing that was noted in the article that I didn't know is that companies have a fiduciary requirement, when they provide a 401k.  I work for a very small company and have been able to successfully request the addition of a relatively low fee index fund.  Has anyone had an experience with bad fund options and how they get changed at a larger company?
