For example, if I wanted to track things I'd like to do every day and how well I did them over time.

For example, if I wanted to read every day, walk 10,000 steps, floss, work out, go to church each week, etc.

Is there any app that would track this and produce a weekly / monthly / quarterly scorecard and let me dynamically create these items and track them?

It's probably not exactly what you're looking for, but I've been using an app called "Goal Tracker" on my Android phone. It lets you create whatever task you want and how often you want to do it (daily, weekly, every other day etc.) and you can have it remind you at a certain time of day. Then you check off each day if you've done it or not.

But there isn't much in the way of producing reports other than giving you a couple of simple numbers (you hit the goal 5/10 times, or you're on a 12-day streak, etc.). It's pretty basic. There is an option to export your data but I haven't tried that. It's possible you could export it into Excel or something to make charts/reports.

I use Way of Life and GoalStreaks on my iPhone. Way of Life produces trend data, so I can see, for instance, that my days of weight lifting per week have increased in the past 6 months. GoalStreaks is a bit simpler and doesn't do trends, but it allows you to create a streak, like not drinking 5 days a week, and it's motivational to try to not break the streak. Both free.
