ACA Exemptions -
Members of a health care sharing ministry - You are a member of a health care sharing ministry, which is an organization described in section 501(c)(3) whose members share a common set of ethical or religious beliefs and have shared medical expenses in accordance with those beliefs continuously since at least December 31, 1999.

Other than accepting medicaid or paying high premiums (compared to non ACA approved plans) what other significant options are there for the early retiree ?

Here's a list of other options.

Other ways to get around the individual mandate include being Native American, Amish, incarcerated, or living abroad.

I'm not Amish - but I did have a cousin that sold their cars and other worldly belongings, turned Amish, and moved to a community in the middle of Missouri .... which in retrospect is pretty mustachian I guess.?

If you are free to pick your religion and free to leave it, then someone who wanted to avoid ACA could become Amish every time that they were asked about ACA . . . and then stop being Amish immediately afterwards.
