5498-SA Incorrect?

Hello All,

I am wondering if my 5498-SA is wrong?

In spring of 2016 I had made a deposit to my HSA for 2015 tax year in order to max it.

On my HSA website it clearly shows a $600 contribution in January of 2016 made for tax year 2015.

For my 2016 contributes it says that I contributed $3455.82 which I expected because I goofed up some of the auto-contribution amounts.

The weird thing is that my 5498-SA for 2016 says that total contributions are $4055.82.

Is this correct?  I thought that the contribution made for tax year 2015 would not be included in my 2016 contributions.

It's correct.  E.g., see 2017 Instructions for Forms 1099-SA and 5498-SA - i1099sa.pdf:

Box 2. Total Contributions Made in 2017
Enter the total HSA or Archer MSA contributions made in 2017.
Include any contribution made in 2017 for 2016.
