Visit amazon.com/mobileapps from your mobile browser, install the Amazon Shopping app, log in to (or create) an Amazon account.

I hadn't set up the app on my new phone yet when Amazon sent me the email, so free $5 for me.

Did you actually log in? When I did it, I had to log into my account.

Yes, and it said then that it was only for new users. It did give me a code I could send to friends, but meh.

I downloaded the app for the first time with your link and even got an email saying I have $5 off but I didn't get a code and when I try to buy something on Amazon it doesn't deduct $5. Any idea what it could be?

I got it to work. I had another promotion with a credit card going but if I take that out I'll be able to use the $5 code.
