Will I Need Legal Assistance for This?

I'm writing a variety of instructional exercises and plan to sell them either on teacherspayteachers.com or other sites.  Part of the exercises has me gathering a 3 page open ended survey from people in a variety of professions.  (Survey is attached).

Do I need any legal documentation from the people who participate in this survey?  I am planning to pay a compendium of $10 in some format- most likely gift cards.

And if I need legal documentation, what type of attorney should I hire??

I am NOT an attorney, but I would suspect you'd want to have them sign a release and or understanding of how the information would be used. Almost like a Privacy Policy statement. Any attorney that deals in contracts (especially consumer contracts) would probably be the place to start. Many attorneys will listen to your needs and if they do not practice the type of law you need, make a referral for no charge. I would start with friends/family and get names of attorney's that are already in you "sphere."
