Supporting friend with serious medical condition

We recently learned an old friend of my husband's (mid-40s) has been diagnosed with a serious medical condition which has an extremely low one-year survival rate.  They do not live near us.  Looking for ideas on how we can support them/let them know we're thinking of them, without intruding.

Contact an immediate relative of theirs and tell them exactly this?

-Contact them and ask if there is any place they would like to visit or anybody they would especially like to come visit them.
-Ask if they would like any help making final arrangements or researching hospice options.
-Send flowers or similar cheery offerings on a regular basis.

When my DH was going through cancer, a dear friend of mine who lived quite far away would message me every 2-3 weeks with a kind thought and tried to open the chsnnels for me to share any struggles with her. I am pretty private when it comes to deeper emotions and she made me feel safe enough to share some things with her. And it just felt good to have someone check in with me semi regularly. Many of my other friends gave me a lot of space ad they worried about intruding but I rarely felt comfortable approaching them even when I needed to.
