Child Care: Serious Safety Occurrences Transparency?

This has nothing to do with Mustachianism, but I love how the board has a global representation.  So I am asking you fellow Mustachians: what is your child care provider's policy pertaining to serious safety occurrences? 
(1) Are ALL parents notified when serious safety occurrences transpire? 
(2) Or is disclosure of serious safety occurrences NOT standard protocol, and only those family/ies involved are in the know?

ETA: Public Health regulations cover off infectious disease disclosure.  I am referring to safety (or lack there of) occurrences.  I changed the title of the thread.  Where I am from there are two forms pertaining to safety occurrences.

Form A: minor occurrence ie. your child scraped their knee. You sign the form, the staff member involved signs the form.  It goes in you child's file.

Form B: serious occurrence covers everything not covered by form A.  Really the sky is the limit here.....use your imagination:)  This form is longer.  It is signed by the Director, Board and parent(s).  The occurrence is serious enough that this form is sent to the Ministry of Education, as daycare is regulated by the government.

So I guess the question should be: if Form B is filled out is this disclosed to all parents, or does only the parent(s) who signed the form know about the violation?

Serious occurrence meaning, what? When I was give my hometown's day camp failed to inform parents that a child had come in with Hand, Foot and Mouth Disease and a couple hundred kids contracted it and the camp had to be shut down entirely that summer. When a teacher at the local after school program was charged with plotting a terror attack EVERYONE knew because it was in all the papers.

I updated the original post.  I also wanted to add if you don't know the answer to the question please find out.  You might be thinking "my daycare is awesome, I never hear anything bad", when in reality, "your daycare might be awesome, but you are not allowed to hear anything bad."

I'm very glad my children have been attending kindergartens and schools where scraped knees only get a band-aid. The oldest have been in two situations that could be described as "major":

1. She was three years old and in a fist figth with a two-year-old. In that figth, she fell and knocked in a few teeth (not out; they disappeared into the gums). The kindergarten called us, we took her to the dentist, and they gave us a form in case she would need dental work as an adult (some kind of insurance policy). They also got rid of the thing with sharp edges that she had knocked her teeth on. It took a while, but both her front teeth had to be pulled because they got infected. The permanent teeth are fine.

2. She was nine, and thought it was a good idea to hang upside down from the railings by the stairs, over the asphalt. It turned out it was a bad idea. Because she was dizzy and had neck pain, we were called and asked to take her to the doctor. Everything was fine, no concussion, but we got an insurance form just in case.

The only time we have gotten information from the school because someone else was injured, was one time a kid broke his leg skiing. They had to call in the ambulance helicopter, and the school thought we should get some facts, not just the tales from the kids.

My vagueness is not helping. Think beyond personal injury, and more towards core safety violations.  ie. Went on an excursion with 10 kids only came back with 9.  Tenth child is rescued by strangers at a busy intersection.  Stranger gets a hold of parents. Are only the parents of the 10th child in the know, or all are parents informed of the violation?

In my state, violations for which the state is aware are posted on the state licensing website.
