Company donation matching policy for donor-advised funds

Hello! Our company recently established a new policy for donation matching. It currently only covers contributions from individuals, but I am working with HR to figure out if we can expand this to donations made from funds as well.

Has anyone worked with an employer who matched donations made from a DAF? If so, can you share some of the details/restrictions/guidelines?

Based on what I've found online, some reasonable guidelines may be:
  • Only donations made out of a DAF to a qualifying 501(c)(3) charitable organization will be considered
  • Donations must be made by the employee (or someone eligible under the company policy)
  • Contributions to the DAF must be made by the employee or eligible party only

Am I missing any loopholes or special considerations there?

My HR does donation match, and I donate from my DAF. 
They only match donations to specific charities they approve. I select a charity from that list, recommend a grant from the DAF. Once it is sent, I send them the receipt from my DAF. It's then matched.
Nothing has ever been questioned about it.

Isn't this already a requirement for any donation made from a DAF?
